If you are interested in starting a new club, please download the forms to the right and email the Student Club Application Form to Woosol Kim, Student Life Advisor. To keep track of club attendance, print out the attendance sheet so you can record and report your club's attendance in order to receive campus involvement points. The Student Club Handbook and all club-related forms can be found on the wall outside of the Student Affairs office on the 2nd floor.
